On or before January 25, 2012, during construction activities, the employer allowed employees to work on a roof that was approximately 28.8 feet high with a 0 pitch and did not ensure that the employees were protected from falls. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, IDENTITY OF PREVIOUSLY CITED STANDARD 29 CFR .501 (b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314456880, CITATION NUMBER 01, ITEM NUMBER 02, AND WAS AFFIRMED AS A FINAL ORDER ON 06/14/10, WITH RESPECT TO A WORKPLACE LOCATED AT 4357 WARREN SHARON RD.,VIENNA, OHIO.
