29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about November 4, 2020, employees were exposed to an amputation hazard while operating a RAS Turbo Bend 6000 System. The employer did not ensure that the point of operation was effectively guarded to protect employees from the hazard of the upper beam clamping and performing a bend on the production piece. The upper beam is actuated with foot pedal allowing the employee to have their hands inside the hazard area.
