29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 24, 2020, employees were exposed to a fall of approximately 18 feet while performing roofing activities on a residential home located at 409 Walnut, Youngstown, Ohio 44505.  The employees were using an extension ladder to access a scaffold and the employer did not ensure that it extended three feet above the landing area.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about August 24, 2020, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 409 Walnut, Youngstown, Ohio 44505.  employees were working on upper levels with tools and materials that could have fallen on the ground employees and the employer did not ensure that they were wearing protective helmets.     Boak & Sons Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100, which was contained in OSHA inspection number 121411, citation number 1, item number 1 and was affirmed as a final order on 3/29/2018, with respect to a workplace located at 17288 W Akron Canfield, Berlin Center, Ohio 44401.

The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.     Boak & Sons Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102, which was contained in OSHA inspection number 1281411, citation number 1, item number 2, and was affirmed as a final order on 3/29/2018, with respect to a workplace located at 17288 W Akron Canfield, Berlin Center, Ohio 44401.  Boak & Sons Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102, which was contained in OSHA inspection number 1268819, citation number 1, item number 1, and was affirmed as a final order on 12/12/2017, with respect to a workplace located at 3557 S Arlington, Akron, Ohio 44312.
