29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about January 24, 2012 the employer did not establish an energy control program that provided periodic inspections and employee training to de-energize and lock out the swaging machine, exposing them to the hazards associated with unexpected startup of the machine during the changing of dies. In accordance with 29 CFR 1903.19(d) abatement documentation is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about January 24, 2012 an employee was exposed to the hazards associated with changing out dies on a swaging machine that was not de-energized and locked out. In accordance with 29 CFR 1903.19(c) abatement certification is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET.
