29 CFR 1910.147(c)(7)(i)(A):     The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.      a. The employer did not train authorized employees, such as supervisors to ensure that they address the isolation and affixing of lockout devices on energy isolating devices during actual servicing and maintenance of machines and equipment being performed by authorized employees.     b. In Department 120, Dip & Bake, on or about January 31, 2012, an authorized employee's training on the purpose and function of the energy control program and knowledge and skills required for safe application, usage and removal of energy controls was inadequate due to the lack of knowledge and safe application of all energy controls while the employee was repairing the recirculating fan on the Conveyor Oven.    Dayton Phoenix Group, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or equivalent, which was contained in OSHA inspection number 312945488, citation 1, item  12, issued August 20, 2009, with respect to this workplace at 1619 Kuntz Road, Dayton.

29 CFR 1910.147(d)(3):     Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).    In Department 120, Dip & Bake,  on or about January 31, 2012, the natural gas was not isolated while an employee repaired the recirculating fan while on top and inside of the Conveyor Bake Oven.    Dayton Phoenix Group, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent, which was contained in OSHA inspection number 321945488, citation 1, item 13, issued August 20, 2009, with respect to this workplace at 1619 Kuntz Road, Dayton.

29 CFR 1910.147(d)(4)(i):     Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.    In Department 120, Dip & Bake, on or about January 31, 2012, lockout devices were not affixed to the electrical #2 (to the exhaust fans) and natural gas energy sources while an employee was on top and inside the oven repairing the recirculating fan on the Conveyor Bake Oven.      Dayton Phoenix Group, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent, which was contained in OSHA Inspection number 312945488, citation 1, item 14, issued on August 20, 2009, with respect to this workplace at 1619 Kuntz Road, Dayton.
