29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; On or about April 30, 2015, in the Tool Room Department on the Chevalier Surface Grinder, Model FSG-3A1224H, the energy control procedure for cleaning the table of the surface grinder did not include specific steps for shutting down, moving and isolating the grinding wheel before affixing a lockout device on the 480 Volt AC electrical disconnect switchbox. Dayton Phoenix Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 190142, citation number 1, item number 2a-2b, and was affirmed as a final order on January 28, 2013, with respect to a workplace located at 1619 Kuntz Road, Dayton, OH 45404.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). On or about April 30, 2015, in the Tool Room Area, a Tool Maker was exposed to a rotating grinding wheel on a Chevalier Surface Grinder, Model 3A1224, while using a water hose to clean the table of metal residue without isolating the 480 Volt AC electrical disconnect switchbox. Dayton Phoenix Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 190142, citation number 1, item number 2a and was affirmed as a final order on January 28, 2013, with respect to a workplace located at 1619 Kuntz Road, Dayton, OH 45404.
