29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. The employer failed to document that the Main Compressor Room and the South Compressor Room complied with recognized and generally accepted good engineering practices. a) The employer did not comply with ANSI/IIAR 2 when it failed to ensure that there were nameplates on compressors 3, 4 and 5 in the Main Compressor Room. b) The employer did not comply with ANSI/IIAR 2 when it failed to ensure that pipes piercing the interior walls of the Main Compressor Room were tightly sealed. c) The employer did not comply with ANSI/IIAR 2 when it failed to ensure that an eyewash and body shower unit was located external to the Main Compressor Room and South Compressor Room. d) The employer did not comply with ANSI/IIAR 2 when it failed to ensure that doors to the Main Compressor Room were tight fitting. e) The employer did not comply with ANSI/IIAR 2 when it failed to ensure that exit doors from the Main Compressor Room and South Compressor Room were equipped with panic-type hardware. f) The employer did not comply with ANSI/IIAR 2 when it failed to ensure that the Main Compressor Room and South Compressor Room were each equipped with two refrigerant detectors. g) The employer did not comply with ANSI/IIAR 2 when it failed to ensure that the Main Compressor Room and South Compressor Room refrigerant detectors provided a visual alarm outside each entrance to the respective rooms. h) The employer did not comply with ANSI/IIAR 2 when it failed to ensure that the Main Compressor Room and South Compressor Room had normal mechanical ventilation that provided at least 20 air exchanges per hour. i) The employer did not comply with ANSI/IIAR 2 when it failed to ensure that there was no airflow to the maintenance shop from the Main Compressor Room. j) The employer did not comply with ANSI/IIAR 2 when it failed to ensure that there were no combustible materials stored in the Main Compressor Room. k) The employer did not comply with ANSI/IIAR 2 when it failed to ensure that the oil drain valve on the York freezer evaporator was equipped with a self-closing or manual quick closing emergency stop valve.
