(a) Vega Construction, LLC @ 2540 Hudson St., Denver, CO 80207: On and before 1/25/12, the correcting employer did not ensure that sub-contracted employees utilized personal fall arrest systems. Sub-contracted employees engaged in roofing activities on a 5 in 12 pitch, single-family, residential roof. This condition exposed employees to a fall hazard of approximately 11ft.
