29 CFR 1910.303(g)(1)(i): The dimension of the working space in the direction of access to live parts operating at 600 volts or less and likely to require examination, adjustment, servicing, or maintenance while alive was less than indicated in Table S1:      West side of Bldg. 977: On January 20, 2012 and at times prior thereto, in the following locations, electrical control and breaker panels were not provided with adequate working clearance due to crash posts located in front of the face of the panels:      A. Panel LP4: The 277/480v circuit panel door only had a nine inch clearance and could not open all the way. This panel energized the west, grid, and exterior lights.    B. Control Panel CP05: The 480v control panel door for the pallet lift systems had only twenty-eight inches of clearance and could not open all the way.

29 CFR 1910.335(a)(2)(ii): Protective shields, protective barriers, or insulating materials were not used to protect each employee from shock, burns, or other electrically related injuries while that employee is working near exposed energized parts which might be accidentally contacted or where dangerous electric heating or arcing might occur:      Bldg. 977: On January 23, 2012 and at times prior thereto, the voltage rated tools provided to the MMHS employees were damaged and the insulation had nicks and slices visible to the naked eye. These tools are used near live conductors and circuit parts up to 600 volts.
