29 CFR 1910.1030(f)(2)(iv):     The employer did not ensure that employees who declined to accept the hepatitis B vaccination offered by the employer signed the statement in appendix A:      a.)  On or about 2/1/12, an SPS employee, hired on 10/1/06, had not signed a Hepatitis B vaccination declination statement.    b.)  On or about 2/1/12, an SPS employee, hired on 10/26/08, had not signed a Hepatitis B vaccination declination statement.

29 CFR 1910.1030(h)(1)(ii)(B):     The medical record for an employee with occupational exposure did not include a copy of the employee's hepatitis B vaccination status, including the dates of all the hepatitis B vaccinations or any medical records relative to the employee's ability to receive vacination as required by 29 CFR 1910.1030(f)(2):    a.)  On or about 2/1/12,the employer did not have a copy of an SPS employee's hepatitis B vaccination status.  The employee was hired on 10/1/06.    b.) On or about 2/1/12, the employer did not have a copy of an SPS employee's hepatitis B vaccination status.  The employee was hired on 10/26/08.

29 CFR 1910.1030(g)(2)(iv):     Annual training for employees with occupational exposure was not provided within one year of their previous training:      a.)  Sterile processing - On or about 2/1/12, an SPS employee had not been provided training within one year of previous training.  Training had been provided in August, 2010 and February, 2012.  The employer had not ensured that annual training was provided within one year of their previous training.

29 CFR 1910.1030(f)(2)(i):     Hepatitis B vaccination was not made available within 10 working days of initial assignment to all employee(s) with occupational exposure:  Note:  Does not apply to employees who have previously received the complete hepatitis B vaccination series, for whom antibody testing has indicated immunity, or for whom the vaccine is contraindicated for medical reasons.      a.) An SPS employee (since 2006) required to clean, disinfect and process contaminated medical equipment such as scopes, IV poles, pumps, electronic monitors, etc, had not been offered the Hepatitis B vaccination.  The employer had not ensured that employees had the Hepatitis B vaccination made available to them after training and within 10 days of initial assignment.    b.) And SPS employee (since 2008) required to clean and process contaminated medical equipment such as scopes, IV poles, pumps, electronic monitors, etc, had not been offered the Hepatitis B vaccination.  The employer had not ensured that employees had the Hepatitis B vaccination made availalbe to them after training and within 10 days of initial assignment.
