Perry Avenue, Chicago, IL - employees installing a water line in a trench measuring approximately five (5) feet six (6) inches in depth were not provided with a safe means of egress, thereby exposing the employees to cave in hazards.     The Garces Contractors, LLC. was previously cited for a violation for this Occupational Safety and Health Standard or its equivalent Standard, 29 CFR 1926.651(c)(2), which was contained in OSHA Inspection Number 77192, Citation Number 1 Item Number 1 and was affirmed as final order on September 20, 2011 with respect to the workplace located at 1832 W. 21st Street, Chicago, IL 60608.

Perry Avenue, Chicago, IL - an employee was injured while installing a water line in a trench. The pavement was undermined at the base of the trench, causing the trench to collaspe onto the employee.     The Garces Contractors, LLC. was previously cited for a violation for this Occupational Safety and Health Standard or its equivalent Standard, 29 CFR 1926.651(i)(3), which was contained in OSHA Inspection Number 77192, Citation Number 1 Item Number 2a and was affirmed as final order on September 20, 2011 with respect to the workplace located at 1832 W. 21st Street, Chicago, IL 60608.
