29 CFR 1926.451(e)(1): Scaffold platforms more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used.     (a) K & K Construction of Weston, Ohio, LLC on or about February 15, 2012, at the address of 1515 W. Wooster St. Bowling Green, OH: The employer failed to ensure a ladder or similar means of access was provided to employees using a welded frame scaffold. The employees were exposed to a fall to the concrete below when utilizing the frame of a masonry walkthrough scaffold to climb to the platforms above.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity):    a)  K & K Construction of Weston, Ohio, LLC on or about February 15, 2012, at the address of 1515 W. Wooster St. Bowling Green, OH: The employer did not ensure employees were protected from a fall while working from a supported scaffold at the west side of a commercial structure. Employees performing overhand bricklaying work from a scaffolding platform without standard railing were exposed to a 13ft fall to the ground below.
