29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a.) K & K Construction of Weston, Ohio, LLC on or about September 15, 2016, at the address of 714 Brittany, Bowling Green, OH: The employer did not ensure a ladder or similar means of access was provided to employees using a welded frame scaffold. The employees were exposed to a fall to the grass below when utilizing the frame of a masonry walkthrough scaffold to climb to the platforms above. K & K Construction of Weston, Ohio, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 191477, citation number 1, item number 1a and was affirmed as a final order on March 15, 2012 with respect to a workplace located at 1515 W. Wooster St. Bowling Green, OH.

K & K Construction of Weston, Ohio, LLC on or about September 15, 2016, at the address of 714 Brittany, Bowling Green, OH: The employer did not ensure employees were protected from a fall while working from a supported scaffold on the chimney of a residential structure. Employees performing tuck pointing work from a scaffolding platform without standard railing were exposed to an approximate 21ft fall to the ground below. K & K Construction of Weston, Ohio, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(vi), which was contained in OSHA inspection number 191477, citation number 1, item number 1b and was affirmed as a final order on March 15, 2012 with respect to a workplace located at 1515 W. Wooster St. Bowling Green, OH.
