29 CFR 1910.147(c)(4)(i):     Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    A.	Vitakraft Sunseed, Inc., located at 20584 Long Judson Rd., Weston, OH: On or about February 13, 2012, the employer failed to utilize machine specific energy control procedures to control hazardous energy on the PSG-LEE Auto-Packer & Auto-Weighter machine (Manufactured by Leepack, Model RP-84TZ, Poly 3).  Employees did not control the hazardous energy while performing activities such as, but not limited to, setting the machine up, making adjustments, cleaning and unjamming it, and performing maintenance on the machine.     B.	Vitakraft Sunseed, Inc., located at 20584 Long Judson Rd., Weston, OH: On or about February 13, 2012, the employer failed to utilize machine specific energy control procedures to control hazardous energy on the Premixer before employees were conducting activities such as, but not limited to, cleaning it.    Vitakraft Sunseed, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(4)(i)), which was contained in OSHA Inspection # 311604946, Citation 1,  Item 4, and was affirmed as a final order on November 6, 2008, with respect to a workplace located at 20584 Long Judson Rd., Weston, OH.     per ISA AVD instance B changed to read:    AVD of instance b amended to read:  		the employer utilized interlocks instead of locking   		out the Premixer for cleaning activities.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Vitakraft Sunseed, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(6)(i)), which was contained in OSHA Inspection # 311604946, Citation 1,  Item 5, and was affirmed as a final order on November 6, 2008, with respect to a workplace located at 20584 Long Judson Rd., Weston, OH. Vitakraft Sunseed, Inc., located at 20584 Long Judson Rd., Weston, OH: On or about February 13, 2012, the employer failed to properly train employees to lockout/tagout the PSG-LEE Auto-Packer & Auto-Weighter machine (Manufactured by Leepack, Model RP-84TZ, Poly 3), used to pack animals food. Employees were conducting activities such as, but not limited to, setting the machine up, making adjustments, cleaning, unjamming, and performing maintenance on the machine without locking and tagging it out.    B.	Vitakraft Sunseed, Inc., located at 20584 Long Judson Rd., Weston, OH: On or about February 13, 2012, the employer failed to train employees to lockout/tagout the Premixer before employees were conducting activities such as, but not limited to, cleaning it. Employees were exposed to the rotating drum of the Premixer.    Vitakraft Sunseed, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(7)(i)), which was contained in OSHA Inspection # 311604946, Citation 1,  Item 6, and was affirmed as a final order on November 6, 2008, with respect to a workplace located at 20584 Long Judson Rd., Weston, OH.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
