B.	Vitakraft Sunseed, Inc., located at 20584 Long Judson Rd., Weston, OH: On or about February 13, 2012, the employer failed to utilize machine specific energy control procedures to control hazardous energy on the Premixer before employees were conducting activities such as, but not limited to, cleaning it.    Vitakraft Sunseed, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(4)(i)), which was contained in OSHA Inspection # 311604946, Citation 1,  Item 4, and was affirmed as a final order on November 6, 2008, with respect to a workplace located at 20584 Long Judson Rd., Weston, OH.     per ISA AVD instance B changed to read:    AVD of instance b amended to read:  		the employer utilized interlocks instead of locking   		out the Premixer for cleaning activities.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Vitakraft Sunseed, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(7)(i)), which was contained in OSHA Inspection # 311604946, Citation 1,  Item 6, and was affirmed as a final order on November 6, 2008, with respect to a workplace located at 20584 Long Judson Rd., Weston, OH.
