29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:     A)    Vitakraft Sunseed, Inc., located at 20584 Long Judson Rd., Weston, OH 43569: On or about September 24, 2014, the employer did not ensure that personal protective equipment such as, but not limited to, personal fall arrest system was used when a maintenance employee was standing on the elevated platforms hand rails. The employee was exposed to a fall hazard of approximately 20 feet from the handrails of the platform to the concrete ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    A)    Vitakraft Sunseed, Inc., located at 20584 Long Judson Rd., Weston, OH 43569: On or about September 24, 2014, the employer failed to ensure that maintenance employee was provided with training on electrical safety-related work practices such as, but not limited to, conducting a test of the volt meter to ensure its proper operation and use of personal protective equipment for the hands, head, and body to protect against electrical hazards while working on or near energized or potentially energized electrical parts. The employee was exposed to 208 volts electrical hazards while verifying the voltage at the electrical panel before re-wiring the starter for the Eriez Vibratory Equipment for the Poly 3 machine.      B)     Vitakraft Sunseed, Inc., located at 20584 Long Judson Rd., Weston, OH 43569: On or about July 15, 2014, the employer failed to ensure that maintenance employee was provided with training on electrical safety-related work practices such as, but not limited to, conducting a test of the volt meter to ensure its proper operation and use of personal protective equipment for the hands, head, and body to protect against electrical hazards while working on or near energized or potentially energized electrical parts. The employee was exposed to 480 volts electrical hazards while connecting/disconnecting programming cable to the PLC in the hay bailer control panel located in Bay 3.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     A)    Vitakraft Sunseed, Inc., located at 20584 Long Judson Rd., Weston, OH 43569: On or about September 25, 2014, the employer failed to utilize machine-specific energy control procedures to control hazardous energy on the Poly 1 Matrix Custom Packaging Machinery (Model # 201815SZXRIS, Serial # 500100MSB01381).  Employees did not control the hazardous energy while performing activities such as, but not limited to, setting the machine up, making adjustments, and cleaning the machine. Employees were exposed to amputation hazards.    Vitakraft Sunseed, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(4)(i)), which was contained in OSHA Inspection # 191515, Citation 2,  Item 1, and was affirmed as a final order on May 21, 2012, with respect to a workplace located at 20584 Long Judson Rd., Weston, OH.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
