29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident: <BR> On or about 14 February 2012, and at times prior thereto, the employees were utilizing the breaker panels for lockout/tagout procedures when the legend for the panels was not filled out or was incorrectly filled out, exposing employees to the hazards associated with an electrical shock or an electrical fire. <BR> a) Breaker Panel-HV for Press 15 had a legend card that was not filled out. b) Breaker Panel DP-2 in the Tool Room/Maintenance Shop had a legend card that was incorrectly filled out.

29 CFR 1910.305(g)(1)(iv)(A): As a substitute for the fixed wiring of a structure; On or about 14 February 2012, and at times prior thereto, employees were utilizing an electrical extension cord to power a Dryer unit within the Utility Room that was approximately 40 feet long and ran above in the open ceiling on top of various pipes and conduit, exposing employees to the hazards associated with an electrical shock or an electrical fire.
