OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause physical harm to employees in that they were exposed to the hazard of being struck by falling loads while lifting and transporting injection mold dies. On or about May 19, 2014, Crane hooks attached to cranes used to lift and move injection mold dies around the facility had missing or damaged safety latches, exposing employees to the hazard of being struck by falling loads.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about May 19, 2014, The Dayton 20 inch drill press in the facility was missing its guard, exposing employees to the hazard of being caught in the rotating part of the press.

29 CFR 1910.305(j)(2)(i): Receptacles were installed so as to require an energized attachment plug as their source of supply: On or about May 19, 2014, a flexible cable was cut and used to power a receptacle box which, in turn, powered a Better Packing tape machine, exposing employees to an electrical hazard.
