29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: On or about October 24, 2019 and at times prior thereto, a Setup Technician in training did not affix their own lockout device on the hydraulic pump lockout switch of an Injection Mold Machine while assisting with injection molding machine setup, exposing employee to hazards of unexpected start-up of energy sources.
