29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:    (a) On or about February 22, 2012, and at times prior, in the powder coat spray booth, also known as the SureCoat line by Nordson, at Adams Thermal Systems, Inc. in Canton, South Dakota, an employee was working with bare hands to operate a hood sprayer to apply powdered coating to thermal systems as they pass through the spray booth.      (b) On or about February 22, 2012, and at times prior, in the powder coat spray booth, also known as the SureCoat line by Nordson, at Adams Thermal Systems, Inc. in Canton, South Dakota, an employee was working with a hood sprayer to apply powdered coating to thermal systems as they pass through the spray booth without appropriate eye protection as the eye protection selected still allowed the black powder coat to get around the eyes.

29 CFR 1910.132(d)(1)(iii): The employer did not select PPE that properly fit each affected employee:    (a) On or about February 22, 2012, and at times prior, in the powder coat booth, also known as the SureCoat line by Nordson, at Adams Thermal Systems, Inc. in Canton, South Dakota, an employee was working with a hood sprayer to apply powdered coating to thermal systems as they pass through the spray booth with a Tyvek suit that was too small for the employee, exposing skin to the black powder coat.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):    (a) On or about February 22, 2012, and at times prior, an employee working in the powder coat spray booth at Adams Thermal Systems, Inc. located at 47920 Fifth Street, in Canton, South Dakota, was exposed to Total Dust, PNOR at levels found by OSHA to be in excess of the OSHA PEL at a Time-Weighted Average (TWA) of up to approximately 59.9 mg/m3, at approximately four times the PEL.      Potentially feasible methods of abatement may include, among others:     1. Conducting a complete evaluation of ventilation system(s), and/or;    2. Prohibiting the partial entry of employees into the spray booth while spraying.
