In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.217(b)(7)(iii): A means of fixing the selection between off, inch, single stroke, and continuous was not capable of supervision by the employer:    a) Triad Capital Group, LLC - Columbia Station, Ohio: On or about February 6, 2012, the employer did not assure that the operating mode on the Bliss 28 press was capable of being set in a fixed position.  The operating mode could be changed by selector switch that was not capable of being secured in a fixed position and supervised by the employer, exposing employees to an amputation hazard.       b) Triad Capital Group, LLC - Columbia Station, Ohio: On or about February 28, 2012, the employer did not assure that the operating mode on the Rockford press was capable of being set in a fixed position.  The operating mode could be changed by a push button without being secured and supervised by the employer, exposing employees to amputation hazards.     c) Triad Capital Group, LLC - Columbia Station, Ohio: On or about February 6, 2012, the employer did not assure that the operating mode of the Minster 50 press was capable of being set in a fixed position and supervised by the employer.  The operating mode could be changed by a selector switch that was not capable of being secured in a fixed position and supervised by the employer, exposing employees to an amputation hazard.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employee(s) were exposed to amputation hazards in that the pedestal was not secured and could be moved and operated directly in adjacent to the hazard area.     b) Triad Capital Group, LLC - Columbia Station, Ohio: On or about February 27, 2012, the employer did not assure the two hand control pedestal used to operate the Warco press #100-2-54C was in a fixed position.  Employee(s) were exposed to amputation hazards in that the pedestal was not secured and could be moved and operated directly adjacent to the hazard area. Employees were exposed to amputation hazards while the press was operated in continuous mode.    b) Triad Capital Group, LLC - Columbia Station, Ohio: On or about February 22, 2012, the employer did not provide and assure that employees operating the Niagara E-60 were protected from the point of operation.  Employees were exposed to amputation hazards while the press was operated in continuous mode.    c) Triad Capital Group, LLC - Columbia Station, Ohio: On or about February 28, 2012, the employer did not provide and assure that employees operating the Rockford press were protected from the point of operation. The original and continuing competence of personnel servicing power presses was not maintained in that:  - Regular inspections of equipment and safeguards, including safeguards that prevent operators from inadvertently reaching into the hazard area of presses were not conducted and/or documented.    - Employees operated presses which were not guarded properly.  - Employees changed dies without die setting procedures and locking the machine out.
