29 CFR 1910.217(b)(13): Control system(s) of mechanical power press(es) were not constructed so that a failure within the system would not prevent the normal stopping action from being applied to the press when required, but would prevent initiation of a successive stroke until the failure is corrected;   a.)     On or about February 7, 2019, the employer did not ensure employees were protected while performing hand in die operations on presses including, but not limited to the T6 South Bend OBI-100 part revolution mechanical power press. Employees are required to operate the press without a brake monitor or anti-repeat function and are exposed to amputation hazards in that the press safety features are not control reliable.

29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment:   a.)     On or about February 7, 2019 the employer did not establish and follow a program of periodic or regular inspections for presses, including but not limited to the T6 South Bend OBI-100 part revolution mechanical power presses to ensure that all parts, including the clutch/brake mechanism, anti-repeat feature and single stroke mechanisms were in a safe operating condition. Employees are required to operate the presses daily and are exposed to crush hazards from press malfunctions that would otherwise be found during inspections, including but not limited to allowing the mode selector switch that doesn't require a key.

On or about February 7, 2019, the employer did not ensure maintenance employees were competently trained before performing work to maintain the T6 South Bend OBI-100 part revolution power press. Employees are required to fix the press when the ram doesn't return to its top position, exposing them to crush hazards.

On or about February 7, 2019, the employer did not ensure employees were adequately trained before starting work operating the presses, including but not limited to the T6 South Bend OBI-100 part revolution mechanical power press. Adequate training was not provided regarding the hazards associated with normal production, including mode selection controls, exposing employees to amputation hazards.
