29 CFR 1926.20(b)(1):     The employer did not initiate and maintain such programs as may be necessary to comply with this part:    On or about March 14, 2012, during construction activities including, but not limited to, roofing activities on a steep roof, the employer did not have a safety and health program implemented at the worksite. The employer did not ensure employees were provided safety and health training or that there was a system in place to recognize and correct unsafe working conditions.

29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or about March 14, 2012, during construction activities including, but not limited to, roofing activities on a steep roof, the employer did not ensure employees were protected from eye hazards by ensuring eye protection was available and worn during nailing and shingling activities.
