29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: a) On or about March 19, 2012, and at times prior, a roofer tied off to a lifelines that was frayed and broken while working on a roof whose eaves were 22 feet above the ground at 18900 E. Main Street in Parker, Colorado. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: (a) On or about March 19, 2012, employees were not trained in the installation and operation of the personal fall arrest system installed for use while working on the roof in that the employees did not adjust the rope-grab up the rope as they moved towards the anchor point.
