In place of this equipment, you may use a water hose that is at least 4 feet (1.22 m) long and at least \3/4\ of an inch (18 mm) thick with a quick-opening valve and carrying a pressure of 25 pounds per square inch (1.62 k/cm2) or less; and    (a)  At the workplace, there were two dip tanks that did not have an eye wash station that was located for immediate emergency use, thereby exposing the employees to eye injuries.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    (a)  At the Carbonless & Cut Sheet Forms, AB Dick 9910XCD serial number 66329 offset printing press did not have guarding installed around the rollers to prevent contact with the ingoing nip points, thereby exposing the employees to caught-in hazards. 29 CFR 1910.215(b)(9):     Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)    (a).  At Carbonless & Cut Sheet Forms, there was a Tradesman 6 inch bench grinder model number 8260 serial number 065549 that did not have a tongue guard installed, thereby exposing the employees to the grinding debris and the parts of the grinding wheel if the wheel were to shatter.

29 CFR 1910.305(g)(1)(iv)(A):     Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:  As a substitute for the fixed wiring of a structure.      (a)  Throughout the facility at Carbonless & Cut Sheet Forms, there were flexible cords used as permanent wiring to supply 110 volt power to equipment.  For example, power was supplied to an overhead extension cord and to a paper counter, thereby exposing the employees to electrical shock and fire hazards.
