OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the installed wood roof trusses did not have the requisite diagonal bracing:  (a) North Side of Building - 23 installed wood roof trusses did not have the requisite triangulated diagonal bracing installed to prevent a collapse of these trusses.

29 CFR 1926.453(b)(2)(v):     A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    (a) South Side of Building - on or about March 26, 2012 - employee working from a Genie S-60 aerial lift at a 26 foot elevation from the ground did not have his body harness attached to the basket of the lift with the required lanyard.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    (a) Third Story Floor, North Side of Building - on or about March 26, 2012 - employee walking the top plate of in interior wall frame at an elevation of 8 feet, 1-1/2 inch from the floor was not protected from falling.
