29 CFR 1910.37(a)(3):     Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.    a. At the Circleville Rural King facility located in Circleville, Ohio, the employer had an exit door located by the delivery trailer in the back room that was blocked with shopping carts.

29 CFR 1910.305(b)(2)(i):     All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    a. At the Circleville Rural King facility located in Circleville, Ohio the employer had an outlet box without a faceplate that was located on the North wall just outside the small engine repair shop.    b. At the Circleville Rural King facility located in Circleville, Ohio the employer had an outlet box without a faceplate that was located on the North wall just outside the small engine repair shop.    c. At the Circleville Rural King facility located in Circleville, Ohio the employer had an outlet box without a faceplate that was located on the South wall just outside the small engine repair shop.    d. At the Circleville Rural King facility located in Circleville, Ohio the employer had an GFCI outlet box without a faceplate that was located on the north wall just inside the small engine repair shop.    e. At the Circleville Rural King facility located in Circleville, Ohio the employer had a light switch box without a faceplate that was located on the north wall just inside the small engine repair shop.

29 CFR 1910.1200(e)(1):     Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:    a. At the Circleville Rural King located in Circleville, Ohio the employer had not developed a written hazard communication program.  Employees at the facility would use chemicals to clean and maintain the facility such as Purple Power, Simple Green, paint and paint thinner.

29 CFR 1910.1200(h)(1):     Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.    a. At the Circleville Rural King facility located in Circleville, Ohio, the employer did not train their employees on the hazards associated with the chemicals that they use or how employees in the facility could obtain material safety data sheets for the chemicals that they use.
