The fire extinguisher was located in the company trailer.  This condition exposed employees to a fire hazard.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that fire extinguisher inspections are up to date.

29 CFR 1910.305(g)(2)(iii):     Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    On or about 03/22/12 the strain relief devices on the following electrical cords were not effective exposing employees to an electrical hazard:         1.  110 VAC to sparking unit, located near the gas flare.       2.  480 VAC power cable to company trailer transformer, located on the drilling rig floor.       3.  110 VAC extension cord plugged into a power strip, located in the top dog house.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that electrical cords are in good conditon.
