29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about March 04, 2015 and at times prior thereto, employees were working on an oil rig site. The 200 AMP power receptacle supplying power to the catwalk was not grounded properly. This exposed employees to electrical shock hazards and/or burns.
