Entire facility - On or about April 2, 2012, the employer's periodic inspections of established energy control procedures were not conducted as required.  Employees who perform servicing and maintenance on such equipment including, but not limited to, the perforating presses were thereby exposed to the hazards associated with the unexpected energization of equipment.
