29 CFR 1910.24(h): "Railings and handrails." Standard railings shall be provided on the open sides of all exposed stairways and stair platforms. Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23. Maintenance Shop - On or about July 23, 2014, the employer failed to guard an open sided staircase leading to a second level which stored parts, thereby exposing employees to 11 foot falls.

Production Department, Press 5 - On or about July 23, 2014, the employer failed to perform a monthly inspection of CM Hoist, Serial # L-37200 with signed certification records, thereby exposing employees to struck by hazards from falling material. b. Production Department, Press 6 - On or about July 23, 2014, the employer failed to perform a monthly inspection of CM Hoist, Serial # L-393A2 with signed certification records, thereby exposing employees to struck by hazards from falling material. c. Production Department, Press 14 - On or about July 23, 2014, the employer failed to perform a monthly inspection of CM Hoist, Serial # 7194BFS, with signed certification records, thereby exposing employees to struck by hazards from falling material. d. Production Department, Press 30 - On or about July 23, 2014, the employer failed to perform a monthly inspection of CM Hoist, Serial # L-707A4, with signed certification records, thereby exposing employees to struck by hazards from falling material. e. Production Department, Press 31 - On or about July 23, 2014, the employer failed to perform a monthly inspection of CM Hoist, Serial # L-2419T, with signed certification records, thereby exposing employees to struck by hazards from falling material. f. Shipping Department - On or about July 23, 2014, the employer failed to perform a monthly inspection of a 7 and 1/2 ton Industrial Crane, with signed certification records, thereby exposing employees to struck by hazards from falling material.

Throughout facility - On or about July 23, 2014, the employer had not performed periodic inspections to ensure that documented lockout/tagout procedures were being followed by authorized employees. Accurate Perforating Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(6)(i), which was contained in OSHA inspection number 329252, citation number 1, item number 1 instance B and was affirmed as a final order on July 5, 2012, with respect to a workplace located at 3636 South Kedzie Ave., Chicago, IL 60632.
