29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) In the Interpretive Center air handling room, a junction box listed and labeled to be installed in the structure of a building was used to construct a conversion plug that allowed a piece of equipment wired with a 20 amp male plug to be powered through a 15 amp receptacle. b) Radio equipment equipped with a 20 amp male plug was powered through a 15 amp receptacle by using the above conversion plug and wiring.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) In the Interpretive Center, air handling and communications room, in the breaker box an opening approximately 5 inches tall by 8 inches wide exposed employees to the live bus bar; b) In the Interpretive Center main office, the base plate at the bottom of the partition was loose exposing employees to the live bus bar. The employer was cited for a violation of the same standard and/or same hazard on 03/09/2011 Inspection #307507285, date 03/21/2011 of Final Order/settlement, and means of abatement.
