29 CFR 1960.59(a): The Agency did not provide appropriate safety and health training for employees:  (a) The employee gaining access to valves was using a 12 foot fiberglass stepladder, which was leaned up against the wall and was not properly setup by fully opening the ladder and extending the spreaders.
