29 CFR 1910.106(d)(3)(ii)(a):     Metal cabinets constructed in the following manner shall be deemed to be in compliance. The bottom, top, door, and sides of cabinet shall be at least No. 18 gage sheet iron and double walled with 1 1/2 - inch air space. Joints shall be riveted, welded or made tight by some equally effective means. The door shall be provided with a three-point lock, and the door sill shall be raised at least 2 inches above the bottom of the cabinet.    a) On or about 4/5/12 in the Ink Mixing Room; the handle of the storage cabinet marked "Glossy" was able to be turned 360 degrees and did not engaged the three point locking mechanism. 29 CFR 1910.147(c)(3)(ii):     In demonstrating that a level of safety is achieved in the tagout program which is equivalent to the level of safety obtained by using a lockout program, the employer shall demonstrate full compliance with all tagout-related provisions of this standard together with such additional elements as are necessary to provide the equivalent safety available from the use of a lockout device.Additional means to be considered as part of the demonstration of full employee protection shall include the implementation of additional safety measures such as the removal of an isolating circuit element, blocking of a controlling switch, opening of an extra disconnecting device, or the    removal of a valve handle to reduce the likelihood of inadvertent energization.    a) On or about 4/5/12 in the Sealing department; employees are turning the "On/Off" switch "Off", removing the key, hanging a tag on the power switch and not disconnecting the air prior to set up and change over of Sealing machine #137 without providing any additional safety measures.      b) On or about 4/5/12 in the Stamping department; employees are turning the "On/Off" switch off, removing the key, shutting of the hydraulics control power, prior to set up and change over of stamping machine #329 without providing any additional safety measures. 29 CFR 1910.147(c)(6)(i)(D):     Where tagout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized and affected employee, of that employee's responsibilities under the energy control procedure being inspected, and the elements set forth in paragraph (c)(7)(ii) of this section.    a) On or about 4/5/12 throughout the facility; employer uses a tagout program when changing over the Roll Feeders and Stamping machines however, all of the annual inspections conducted in 2011 (four) were performed on the blade changing in the Bindery department and only with authorized employees using the cutter. 29 CFR 1910.335(a)(1)(i):     Employees working in areas where there are potential electrical hazards shall be provided with,and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.    a) On or about 4/18/12 throughout the facility; employees are involved in using a voltage meter to test for the presence of electricity at 208 volts AC without the use of electrical protective gloves, hard hat, protective clothing and eye protection against the possibility of arc flash hazards.
