29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from fallingor flying objects, or from electrical shock and burns, shall be protected by protective helmets. Employee(s) did not wear a hardhat for overhead protection while working in a trench. The employees were exposed to struck-by hazards from falling tools, dirt, and/or rocks.

29 CFR 1926.651(c)(2): Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. The employer failed to ensure that the trench employee(s) were working in had a sufficient ladder for safe access and egress of the trench. The trench was 4 feet 11 inches deep, 7 feet wide, and 18 feet 6 inches long. Lack of a ladder exposed employees to a fall hazard and not having a safe means of escape in case of an emergency.
