29 CFR 1926.652(c)(2)(i): design of support systems. shield systems, or other protective systems that are drawn from manufacturer's tabulated data shall be in accordance with all specifications, recommendations and limitations issued or made by the manufactuer. On or about December 12, 2012, an employee was observed standing on a ladder in a trench box within a 14 foot deep trench. The north wall of the trench was separated from the trench box by a space of approximately 2 feet and the south wall of the trench was separated from the trench box by a space of approximately 8 feet. This exceeded the allowance established by the "Speed Shore DW model" tabulated data, Section 7.6 that states the trench walls cannot exceed the trench shields width by more than 12 inches.

29 CFR 1926.652(g)(1)(iii): Employees shall be protected from the hazard of cave-ins when entering or exiting the areas protected by shields. On or about December 12, 2012, an employee was observed standing on a ladder that was positioned inside of an 8 foot high trench box. This box was resting within a 14 foot deep trench. The ladder could not bridge the height and separation from the trench box to the nearest egress point at the top of the north wall. There was a void space between the trench box and trench wall of approximately 2 feet on the north side and 8 feet on the south side. The employee could not exit the trench without assisstance from other employees standing at the edge of the north wall.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. On or about December 12, 2012, on the west side of Old Jefferson Hwy, Prairieville, LA, an employee was observed standing on a ladder within an 8 foot high trench box that was placed within a 14 foot trench. The employee was adjusting insulation sheeting to prepare for a sewerage pipe installation. The employee was directly exposed to a potential collapse of the north wall.
