This conditioned exposed employees to contusion hazards in the event the equipment destabilized during use.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replace: Maintenance Shop: On or about April 11, 2012 and at times prior thereto, the insultation on the work return ground lead of a Lincoln Electric Vantage, Equipment Number WLD-5003 arc welder was damaged and exposed to the conductor. This condition exposed employees to electrical shock hazards in the event of contact with the exposed conductor.
