29 CFR 1910.37(a)(2): Exit routes must be arranged so that employees will not have to travel toward a high hazard area, unless the path of travel is effectively shielded from the high hazard area by suitable partitions or other physical barriers. North Star Island, Welding shop area: On or about 19 March 2014 and times prior thereto, three 25-pound propane tanks used for pre-heating during welding operations were stored outside on the exit door landing. This condition exposed employees to delays in exiting the building resulting in smoke inhalation and/or fire hazards.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch. North Star Island, Welding shop area: On or about 19 March 2014 and times prior thereto, the Dayton 10" Bench Grinder, Lot No. 1210057 work rest measured 3/8 of inch from the abrasive wheel. This condition exposed employees to a struck by/caught in hazard.
