29 CFR 1910.106(e)(6)(i):  Adequate precautions were not taken to prevent the ignition of flammable vapors:  Liquid Molding: Employees dispensed Dow Corning 997(R) Varnish, which has a flash point of 77 degrees Fahrenheit, toluene and/or xylol into a plastic five gallon container for mixing. The resulting mixture was then poured into a metal vessel. Neither the container, nor the vessel were grounded.

29 CFR 1910.106(e)(6)(ii):   Class I liquids were  dispensed into containers without the nozzle and container electrically interconnected:  Liquid Molding: Employees dispense Class 1B, Dow Corning 997(R) Varnish, Toluol and/or xylol  from a metal five gallon container into a plastic five gallon container for mixing. The resulting mixture is then dispensed into a metal vessel. Neither the metal containers, mixing containers, or the vessels were bonded.

29 CFR 1910.303(g)(1)(vii)(A)(2): The space equal to the width and depth of the equipment was not kept clear of foreign systems to avoid damage from condensation, leaks, or breaks in such foreign systems. This area shall extend from the top of the electric equipment to the structural ceiling:  Main Electrical Room:  The varnish system's vacuum pump exhausted vapors from flammable liquids through polyvinyl chloride piping. The piping was located between the top of the electrical equipment and the structural ceiling. The vapors were from Dow Corning 997(R) and BC346, both of which are flammable coatings.

29 CFR 1910.304(g)(5):  The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  Liquid Molding Department: A Plason Manufacturing Company work lamp, mounted to a two foot by two foot dip tank, was missing its ground pin, thereby exposing employees to electric shock hazards. The tank contained residual amounts of Dow Corning 997(R) Varnish which had a flash point of 77 degrees Fahrenheit.

29 CFR 1910.125(e)(1)(i): All electrical wiring and equipment in each vapor area and any adjacent area did not conform to the applicable hazardous (classified)-area requirements of subpart S of this part (except as specifically permitted in paragraph (g):   Liquid Molding:   A Plason Manufacturing drop light mounted on the vacuum coating tank was used for illumination during the varnish coating process.  Dow Corning 997(R) Varnish had a flashpoint of 77 degrees F and the light was located in the Class I area around the tank.

29 CFR 1910.125(e)(4)(i):  All vapor areas were not kept free of combustible debris and were not kept as free as practicable of combustible stock:  Liquid Molding:   A wood plank was used as a cover for the vacuum coating tank.  Dow Corning 997(R) Varnish which has a flashpoint of 77 degrees Fahrenheit and BC346 baking varnish which has a flashpoint of 45 degrees Fahrenheit was used in the coating process.
