Section 5(a)(1) of the Occupational safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:     a)Employees working at drilling rig #1 were exposed to struck by hazards in that the wire rope used to support the drilling tongs was not installed with enough wire rope clips for the size of wire rope used. Two wire rope clips were used.    AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS: Install a minimum of three wire rope clips when the size of the wire rope is greater than 7/16 inch as recommended in Chapter 4 of the Wire Rope Users Manual from the Wire Rope Technical Board.     b) Employees working on drilling rig #1 were exposed to struck by hazards from the rig structure tipping in that the wire rope used for the 2 guy wires on the drilling floor were not installed with enough wire rope clips for that purpose. Two wire rope clips were used.     AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS: Install a minimum of three wire rope clips as recommended by API RP 9B Recommended Practice on Application Care and Use of Wire Rope for Oilfield Service, 11th Edition 2002.

1910.157(d)(1): Portable fire extinguishers were not  selected based on the classes of anticipated workplace fires and on the size and degree of hazard which would affect their use:    Employees engaged in drilling operations at Rig #1, located at Sec 18, Twp. 26, Row 5, El Dorado, KS were exposed to fire hazards in that the number of portable fire extinguishers was not adequate for the type of workplace.  There was only one fully charged ABC portable fire extinguisher available at the worksite.        NOTE: API RECOMMENDED PRACTICE 54; 7.2.4 Drilling rigs shall have readily accessible, in good operating condition, not less than four(4) 20-pound capacity fire extinguishers with a Class BC rating.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    Employees working at Rig #1, in the area of Continental Emsco D-300 mud pump were exposed to struck by hazards of moving parts of the pony rod.  Two 10 X 20 inch openings were not guarded.

29 CFR 1910.219(d)(1):  Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    Employees working at Rig #1 were exposed to amputation hazards in that pulleys located less than 7 feet from the working level were not guarded in the following instances:    a)	  Mud pump motor.  b)	 Alternator for the motor on the drill floor.
