29 CFR  1910.29(b)(2)(i) Midrails were not installed at a height midway between the top edge of the guardrail system and the walking-working surface:  Employees engaged in activities near the walking/working surface at the mud pits were exposed to fall hazards in that the mid rail was missing. At the mud pit walking/working area, the guardrail was missing between the top rail and the bottom of the platform, exposing 40 inches of unprotected space directly over the mud pit.

29 CFR  1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  The employer is failing to protect employees from fall hazards. This was most recently documented on or about November 8, 2022 where employees are required to work at heights in excess of 6 feet from the ground without fall protection training. Employees work at heights that exceed 50 feet from the oil rig platform to the rig floor below and are doing so without being trained by a qualified person prior to beginning work at heights.

29 CFR  1910.303(g)(2)(i):Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:  Employees engaged in activities near the breaker control panel located in the doghouse room were exposed to shock hazards in that exposed and energized electrical equipment was not guarded.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees engaged in oil and gas extraction activities on the deck of Rig #2 near the operator's station was exposed to caught-in and struck-by hazards in that the rotating shaft and pulley located on the draw works was not guarded.   The C & G Drilling Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1910.212(a)(1), which was contained in OSHA inspection number 1316078, citation number 1, item number 6 and was affirmed as a final order on 9/21/2018, with respect to a workplace located at Sec 1, Twp 3, R5 near Winfield, KS 67156.

29 CFR  1904.39(a)(1):29 CFR 1904.39(a)(1):The employer did not report the in-patient hospitalization of an employee within twenty-four (24) hours to the Area Office of the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor, that is nearest to the site of the incident.  A work-related incident which an employee was seriously injured due to a fall from an Oil Derrick was not reported to OSHA within 24 hrs. The incident occurred on October 30, 2022, at around 1645 hrs. and was not reported with 24hrs.
