29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about April 16, 2012, on the east side of the building, an employee engaged in carpentry activities on a commercial building utilized an extension ladder that did not extend 3 feet above the landing to access the upper landing was exposed to the hazard of falling approximately 19 feet to the lower level.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about April 16, 2012, on the east side of a commercial building, an employee engaged in carpentry activities on a commercial building without fall protection was exposed to the hazard of falling approximately 19 feet to the lower level. The Mountain States Framing, LLC was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent Standard 29 CFR 1926.501(b)(1) which was contained in OSHA Inspection number 313413114, Citation number 01, item number 002 and was affirmed as a final order on 10/27/1009, with respect to a workplace located at 4740 Bromley Ln., Brighton, CO 80601.
