29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths ofwork and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall haveinsulation adequate for the service conditions.[55 FR 13709, Apr. 11, 1990; 61 FR 9227, March 7, 1996; 70 FR 53929, Sept. 13, 2005] Welding shop: On or about 11 April 2012, and times prior thereto, the electrical lead on the Miller XMT 350 CC/CV welder had exposed wires exposing employees to electrical shock hazards.

29 CFR 1926.351(b)(4): Cables in need of repair shall not be used. When a cable, other than the cable lead referred to inparagraph (b)(2) of this section, becomes worn to the extent of exposing bare conductors, the portion thus exposed shall be protected by means ofrubber and friction tape or other equivalent insulation. Arctos Ramp: On or about 11 April 2012, and times prior thereto, welding cable on Lincoln Electric welder had of exposed wires. This exposed employees to electrical shock hazards.
