ALLEGED VIOLATION DESCRIPTION (A.V.D.):     (a) 13 Dulles Drive Camp Hill, PA - Treatment tank two on the south side of the facility had a service panel with 110 volt  live wires exposed and connected with wire nuts on the ground that were not protected with a cover or an enclosure, thereby exposing employee(s) to an electrical shock, on or about April 23, 2012.    No abatement certification or documentation required.
