29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Mount Holly Springs, Mill Street Wste Water Treatment Plant - The employer did not maintain the egress ladder inside of the protective system while employees worked in a 10 foot 6 inch deep trench on or about May 23, 2013. No abatement certification or documentation required.

29 CFR 1926.652(c): 29 CFR 1926.652(c): Support systems, shield systems, and other protective systems were not designed and constructed in accordance with the requirements of paragraph (c)(1-4); as applicable: (a) Mount Holly Springs, Mill Street Waste Water Treatment Plant - The employer did not select and utilize a protective system that was suitable for the location and conditions of the excavation/trench and were exposed to cave in and crushing hazards, on or about May 23, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.652(g)(2): Excavations of earth material to a level no greater than 2 feet (.61m) below the bottom of a shield was permitted: (a) Mount Holly Springs, Mill Street Waste Water Treatment Plant - Men worked in a 10 foot 6 inch deep trench inside an 8 foot trench box, the soil was excavated to a depth of 2 feet 7 inches below the bottom of the trench box, and employees were exposed to the hazard of a collapse of the adjacent soil below the bottom of the trench box on or about May 23, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
