29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Workplace, Employees were using the Heat Tunnel, Stork Texwrap, Model # T1322-03, Serial #D1232, without a guard on the entrance of the wired conveyor belt.  b) Workplace, Employees were using the Heat Tunnel, Stork Texwrap, Model # T1322-03, Serial #D1232, without a guard on the exit of the wired conveyor belt.  c) Workplace, Employees were using the Heat Tunnel, Stork Texwrap, Model # T1322-03, Serial #D1232, without a guard on in-going nip point, of the exit.    Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):      a) West side, Sealer Department, L-Sealer Machines #1, #2, #3, #4 off the aisle, did not have pulley guards on an approximately 3" diameter pulleys, driven by 1/2" belts, located approximately 35" off the floor. Employee caught his finger in the in-going nip points, of machine # 2; On or about 04/25/12.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
