Production Facility - On or about March 28, 2023, the employer did not assess the workplace to determine if hazards were present which necessitated the use of PPE. Employees were required to use PPE in certain areas of the facility; however, the employer had not determined if the PPE being provided to employees was adequate through a hazard assessment.

Shipping and receiving departments - On or about March 28, 2023, employees tasked with using a personal fall protection system while working from an elevated lift basket on the forklifts were not trained on the nature of the fall hazards, the procedures to follow to minimize those hazards,  or the correct inspection and use of the personal fall protection system used.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  Instance a):  Thiele box Forming machine on kit line 1 & kit line 2 - On or about March 28, 2023, employees were exposed to ingoing nip points where the edge of the conveyor belt was exposed where it goes over the roller at the union with the middle conveyor. An employee suffered injuries when their fingers were pulled into the conveyor at this location.  Instance b):  Middle conveyor on kit line 1 & kit line 2 - On or about March 28, 2023, employees were exposed to ingoing nip points where the edge of the conveyor belt was exposed where it goes over the roller at the shrink wrap machine end of the conveyor.  Instance c):  Shanklin Sealer machine on kit line 1 & kit line 2 - On or about March 28, 2023, employees were exposed to rotating parts on the ingoing conveyor at the union with the middle conveyor, where there was unguarded gearing on the conveyor roller.  Instance d):  Finisher machines 1 through 6 - On or about March 28, 2023, employees were exposed to rotating parts at the point of operation while brushing paint rollers, where the entire face of the brush drum was open and unguarded and employee's fingers were within 4 inches of the rotating brushes.
