29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:    The employer does not ensure that the floor of every workroom in the facility is maintained in a clean and , so far as possible dry condition.     a) This violation was observed on or about April 27, 2012, in the Auto Grind Area, where employees who were exposed to slips, trips, and fall hazards, were working and walking on floors with metal dust accumulation.     b) This violation was observed on or about April 27, 2012, in the Wet Grind Area, where employees who were exposed to slips, trips, and fall hazards, were working and walking on floors with metal dust accumulation and water.

This violation occurred on or about April 27, 2012, in the facility where employees were allowed to perform service and/or maintenance including set-up on coil making machines, manual horizontal lathes, surface grinders, coil spring grinders, wet grinders, power presses, peening machines, and other industrial equipment.

COILING TECHNOLOGIES, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.212(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311360325, CITATION NUMBER 1 ITEM 3 AND WAS AFFIRMED AS FINAL ORDER ON 11-8-2007, WITH RESPECT TO A WORKPLACE LOCATED AT 7777 WRIGHT RD., HOUSTON, TX 77041. FOR THIS CITATION ITEM, THE FINAL ORDER DATE WAS THE DATE BOTH PARTIES SIGNED THE INFORMAL SETTLEMENT AGREEMENT.

29 CFR 1910.215(a)(4):     Grinding machinery was not used with work rest(s) to support offhand grinding work:    The employer does not ensure that the work rests of grinding machines are kept adjusted to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and rest, which may cause wheel breakage. This violation was observed on or about 4-26-12, in the Maintenance Area, where employees who were exposed to struck-by hazards were using a bench top grinder with a work rest that had a gap greater than 1/8th inch.
