Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, was not constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel:   (a) In the golf course maintenance shop on or about May 10, 2012, and at times prior thereto, there was no tongue guard for an abrasive wheel on a Dayton 6 Variable Speed Bench Grinder.  The wheel housing was about one half inch from the abrasive wheel.  The wheel was used to grind lawn mower blades.
