29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, was not constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel: (a) In the golf course maintenance shop on or about May 10, 2012, and at times prior thereto, there was no tongue guard for an abrasive wheel on a Dayton 6 Variable Speed Bench Grinder. The wheel housing was about one half inch from the abrasive wheel. The wheel was used to grind lawn mower blades.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc: (a) In the golf course maintenance shop on or about May 10, 2012, and at times prior thereto, the Dayton 17 inch Drill Press, model number 3Z9184, did not have a guard for the chuck and drill.

29 CFR 1910.212(b): Machines designed for a fixed location were not securely anchored to prevent walking or moving: (a) In the golf course maintenance shop on or about May 10, 2012, and at times prior thereto, the Dayton 17 inch Drill Press, model number 3Z9184, base was not anchored. There were slots in the base to anchor it. (b) In the golf course maintenance shop on or about May 10, 2012, and at times prior thereto, the Dayton 6 inch Bench Grinder was mounted on a pedestal that was not anchored to the floor. There were holes in the pedestal's base to anchor it.
