29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Quick Tab II, Inc., Tiffin, Ohio: On or about May 9, 2012, the employer failed to develop a lockout program for the control of hazardous energy, which includes machine specific procedures, training and periodic inspections while performing servicing and maintenance activities on printing equipment, such as, but not limited to the Stevens manufactured 2 color and Kluge manufactured (Serial #155EP234) printing presses. Employees were observed cleaning the blanket of the press without isolating all sources of hazardous energy, exposing them to caught-in and crushing injuries in the event that the equipment were to move unexpectedly.
