29 CFR 1926.102(a)(1):     Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:    JOB SITE: 3956 Triple Crown Drive, St. Louis, MO 63034, at this work site, workers were observed using pneumatic nail gun installing new roof shingles, without any form of eye or face protection.  The workers were exposed to a struck-by hazard.    Above the Rest Roofing & Exteriors was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 315979708, citation 1, item 2, issued on 12/05/2011, and became a Final Order on 01/02/2012.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:    JOB SITE: 3956 Triple Crown Drive, St. Louis, MO 63034, at this work site, workers were observed installing shingles on a two story residential home without any type of fall protection.  The eave to ground level from the 6/12 pitch roof was approximately 17 feet, 8 inches.      Double Repeat:    Above the Rest Roofing & Exteriors was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315814111, citation 1, item 1, issued on 08/18/2011, and became a Final Order on 09/12/2012.    Above the Rest Roofing & Exteriors was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315979708, citation 2, item 1, issued on 12/05/2011, and became a Final Order on 01/02/2012.
